Our medical service includes comprehensive physical examinations, womens' health, and chronic medical conditions, such as diabetes, high blood pressure, high cholesterol, heart disease, anemia, thyroid disease, arthritis, osteoporosis, prostate disease, GERD, immune disease, sleeping disorders, allergy, depression, anxiety, infectious disease, ADHD, injury, skin disease, obesity, obstructive sleep apnea (OSA), and travel medicine. We are also experienced in skin procedures, including the removal of moles, warts, cysts, and ingrown toenails. Cryotherapy, excision, suturing, and wound care are also available.

If possible, please type in your info, print, and bring the completed I-693 form to your visit, using black ink. You will need to fill out only page 1 and the first line of pages 2-13. Otherwise, you will need to fill out the I-693 in our office.

Currently all pilot applicants must log into the FAA site MedXPress (weblink) and complete the form. You will be issued a comfirmation number. In order to serve you in a timely fashion, please let us know your confirmation number before your visit so we can prepare.
